How to make an appointment

Enter your details in the below webform or contact one of our Booking Agents on the below details to ask for a Doctor or Nurse medical to be done at a venue of your client's choice. 

 

Please be sure to have the following information at hand when requesting an appointment:

 

• Policy / Proposal Number

• Client's name and ID Number

• Street address of home or workplace 

• Contact numbers (an alternate contact number is recommended)

• Doctor or Nurse Medicals as required by Underwriting

• Your client will be requested to provide a form of Identification  on the day of the appointment

•  Client identification verification via ID, Passport or Drivers License is mandatory before carrying out any medical procedures.
